A series of tailored fact sheets for GPs or other health professionals to give out to patients to encourage them to make a quit attempt and use the stop smoking services. There are 12 fact sheets: one for each of the major conditions relevant to smokers (e.g. high blood pressure, asthma, pregnancy etc). The fact sheets are tailored to your PCT, acute hospital trust or organisation, with its logo and contact details and any other information you want included. The fact sheets are provided as PDFs so that they can be printed on demand. The price includes updates for 12 months if you find you need changes made. The cost for 12 tailored fact sheets mainly covers the administrative and design work involved in tailoring. Please contact us for samples or to discuss any other specific needs. It is possible to have fewer than 12 fact sheets.
